Artificial intelligence incidents are becoming increasingly common, but there’s no unified system in place to report and analyze them. The Center for Security and Emerging Technology proposes a comprehensive framework for mandatory AI incident reporting, combining lessons from sectors like healthcare, transportation, and cybersecurity. This approach would ensure that incidents, ranging from harmful AI malfunctions to near misses, are consistently documented, allowing for better risk assessment and AI safety measures. By adopting standardized reporting, governments and organizations can enhance transparency, foster accountability, and build more trustworthy AI systems.
